USPS has updated PS Form 3971, Request for or Notification of Absence.
Postal Service employees who plan to take leave should use only the October 2017 version of PS Form 3971, Request for or Notification of Absence.
All earlier versions of this form are outdated and should be recycled.
This change took effect Nov. 1.
The current PS Form 3971 forms included updated Hatch Act and Privacy Act statement information.
To download the form, go to the PolicyNet site and select the “Forms” tab on the front page.
The forms can be also ordered from the Material Distribution Center or through eBuy2 under the MDC eBuy2 catalog.
Use PSIN PS3971 and PSN 7530-02-000-9136 to order the forms.
